WHO CAN WE PROVIDE CARE FOR?

GV Hospice provides a free, in-home, specialist palliative care service to support people with life limiting illness and their families.

GV Hospice provides in-home specialist Palliative Care to people who meet the following criteria:

  1. Living with a life limiting illness
  2. Live within the Greater City of Shepparton.

OUR SERVICES

SYMPTOM SUPPORT

GV Hospice has a team of specialist palliative care nurses to provide support for symptom management.

Support is provided by phone or home visits Monday to Friday. There is also a nurse on call 24/7 for emergency situations.

Our focus is to provide symptom management through assessment, planning, education and specialist clinical care to enhance quality of life for our clients.

NAVIGATE YOUR JOURNEY

Our specialist palliative care clinicians at GV Hospice can provide consultancy, support and advice to GP’s and the Care Team.

The primary medical care is provided by the client’s GP.

GV Hospice work with the care team closely in order to maintain quality of life for our client and provide coordinated care.

GV Hospice is here to assist in advocacy and support the client in navigating their journey.

EQUIPMENT LOANS

GV Hospice have a large store of equipment that can be loaned free of charge to our clients, this is made possible by the continuing generous support of the local community. Items include;

  • Over Toilet Seats
  • Electric Lift recliners
  • Wheelie walkers
  • Wheelchairs
  • Commode
  • Oxygen concentrators
  • And more…..

NURSE-ON-CALL

GV Hospice, supported by community donations, offers 24hrs a day, 7 day a week on call nurses.

This means clients and carers can phone at any time day or night with emergency concerns and speak with a specialist palliative care nurse from the GV Hospice team.

CLIENT CARE VOLUNTEERS

Our volunteers make a difference to the lives of our clients and their families. The companionship that develops can have a positive impact on client and carer journeys.

Individual needs and circumstances are assessed to make the best match between client and volunteer.

Volunteers can help in a variety of ways usually for around an hour a week including;

  • Taking clients on a drive
  • Attending appointments
  • Out for a coffee
  • Activities like reading or fishing
  • Allow carer’s to have a break or run errands

FAMILY SUPPORT TEAM

Our Family Support team comprises of bereavement counsellors, family support workers and carer wellbeing co-ordinators.

The supportive care team provide support to clients, carers and families. Services include;

  • One-on-one or group counselling
  • Navigating and linking of additional services
  • Carer wellbeing activities
  • Support writing Advanced Care Directives
  • Referring clients/carers to other service providers

GV Hospice provides symptom management and psychological support to enhance a person’s quality of life when living with a life limiting illness or terminal illness.

We provide support for family/carers.

We are not funded to provide personal care.

Anyone requiring specialist palliative care can be referred to GV Hospice provided they live within the Greater City of Shepparton Local Government area and meets the below criteria;

  • Client or their legal decision maker is aware and consents to a palliative care referral.
  • The client’s life-limiting condition requires symptom control and care to enhance quality of life.
  • Identified needs that are no longer being adequately addressed by current services
  • Client has complex symptom management needs ( This may include management of symptoms during oncological treatment with curative intent)
  • Client has complex and rapidly changing physical symptom management (pain, nausea, dyspnoea, fatigue etc).

In addition to completing the online referral;

Please provide copies of all relevant medical information e.g. health summary, current medication list, recent pathology, scans, relevant specialist letters, encounter summaries.
