Registered Nurse – Community Palliative Care Specialist

Are you a compassionate and skilled Registered Nurse with a passion for delivering high-quality palliative care in the community?

We are seeking a dedicated Registered Nurse to join our Community Palliative Care team on a 0.6 EFT basis. This rewarding role provides specialist palliative care support to clients and their families, enabling people to receive care and dignity in the comfort of their own homes.

About the Role

As a valued member of our multidisciplinary team, you will:

  1. Deliver person-centered palliative care to clients with life-limiting illnesses,
  2. Provide clinical assessment, symptom management, education and support to clients, families and carers,
  3. Collaborate closely with general practitioners, specialists, allied health professionals and community service providers,
  4. Participate in care planning and contribute to the delivery of holistic, evidence-based care.

Essential requirements:

  1. A Registered Nurse with current registration with AHPRA.
  2. Experienced in delivering high-quality clinical care, preferably in community, palliative care, oncology, aged care or a related setting.
  3. An excellent communicator with strong assessment and care coordination skills.
  4. Committed to providing compassionate, client-focused care.
  5. Specialist qualifications in Palliative Care, or
  6. Completion of a postgraduate qualification within an agreed timeframe, with organisational support provided.
